Dear Mr. Bradley,
Every dental office is going to charge you for flouride when it's is not covered under your dental insurance. It is separate from the cleaning. What you failed to understand is that the agreement that we were wanting you to sign is that you will pay your dental bills if your insurance does not cover your services. What the insurance company does is out of our control. Just because we are participating providers does not mean that will just write off your services if your insurance company does not pay.
I'm sorry that you have chosen to try out another dental office. If you would like to have your records transferred please stop by our office to sign a records release form.
