I was sent a surprisingly large amount of paperwork to be filled out prior to the appointment via email. The most frustrating part of that is it was made very clear that the new patient (me) could not leave the page or would have to start over as a different patient (???).
Also, was given the address and when I showed for my appointment, apparently this practice had moved with not notification to new patients. Thankfully, Dr Kostakis’ given address is a new and completely different practice.
When I called to find out why I was directed to the wrong dental office, I was hung up on by the automated system multiple times.
So I now have a fabulous new dentist and Kostakis’s office lost a new patient, and I’m sure I’m not the first!

We do not send forms to any of our patients as it is very easy to access through our website. Also we have not moved our practice, we have been diligently serving from the same location for over 29 years, and we do not use an automated system.
