To request an appointment within 5 business days please call (419) 228-3338. Appointment times and Monday, Tuesday & Thursday 7 am -11:30 & 1 pm - 3 pm Friday 7 am-12 pm. Our latest appointment of the day is 3 pm Monday, Tuesday and Thursday. We do not offer later appointments. We do not see patients on Wednesday as doctor is not in the office. We do not accept Buckeye Insurance. State funded insurance is accepted for children 18 years of age and under.
