Request an Appointment

Please remember that this is a REQUEST for an appointment not a booked appointment.  We will respond with a confirmation or a rescheduled appointment time that works for our schedule.  Please also realize that for the most popular appointment times we are booked well in advance, though openings do present themselves on occasion.  Failure to cancel or change an appointment before our 24 hour limit may result in a fee.  Thank you.
