Our staff is dedicated to making your first visit to our practice a positive experience. When you contact our office to schedule your initial appointment, your questions, requests and concerns regarding your care will be heard, understood and accommodated. After selecting an appointment with our doctors, you have the option of receiving your new patient forms by mail, email or accessing them on our website. Easy directions to our offices will be included with these forms. We are conveniently located on the ground level of the Cummings Center Professional Building complex and offer complimentary parking to all of our patients.
