The visit was fantastic. The pre-paper work took me over an hour to complete. If I was a new patient I may accept I would need to fill out multiple pages. But with me being a patient for at least 4 years, they should have all of this data on file. This part of the total experience needs to be approved upon.

Thank you so much for your feedback! You are correct in that an existing patient should only need to update their medical history once per year. Updating your medical history is necessary in order to keep to the high standard of care that Dr. Berman provides for his patients. We will use your feedback to communicate to patients that only the medical history form is necessary.
