On Monday October 22, 2012 I had an appoinment for a teeth cleaning.
However, on my previous visit (scheduled for a teeth cleaning) my teeth were not cleaned. Instead, a review was performed and it was recommended that I have a perio-maintenance plan performed and it would cost me out-of-pocket, approximately $1,600 (or so). At that time I declined since I did not have the "out-of-pocket" expense.
Om Monday October 22nd another perio-eval was performed taking up a large part of the one hour visit (intended for a simple teeh cleaning). After that review I was presented with a written waiver and release of claims pursuant to my declination of a full-up perio-maintenance plan. I signed the waiver. They proceeded then to clean my teeth which took approximately 10-15 minutes.
However, upon departure I was informed my out-of-pocket expense for the teeth cleaning was $100.00, because it was some kind of "deep cleaning." However, the cleaning was only on the face of the tooth and not deep.
I was informed that the bill was going to be submitted to Delta Dental, as well, but that my share would be $100.00. Please provide me with all insurance claim filings for this visit and any response you receive from Delta Dental. And, also please provide me with the immediately preceeding visit, insurance filings as well.
We will honor this request and in the future please contact us directly via our web site for immediate action items so we can help you get resolve with any insurance issues with your insurance provider.
