I'm a longtime client of SMAH. On my recent visit I was asked to fill out some kind of information sheet about my visit. There were questions about my dog that should be on his chart already since SMAH has been the only vet to care for my pet. Furthermore, there was a question about additional requests for the visit. I added two items and they weren't addressed. That form was a waste of time and I hope they discontinue using it.
Generally I'm very happy with SMAH but provide this negative feedback as a service to them as a customer. Other than this they provide excellent service
